Chronic Disease Management

The Fono offers self-managed support for chronic diseases including heart disease, diabetes, stroke, cancer and chronic obstructive pulmonary disease.

From middle age onward, chronic diseases are the major causes of avoidable mortality (Ministry of Health 2004).

Pacific adults have higher rates of avoidable mortality than the ‘all New Zealand’ benchmark and on average, live about eight years less than the general population. 

Common conditions such as cardiovascular disease, diabetes and stroke are significantly more prevalent among Pacific peoples than among the general population (Ministry of Health).


Asthma Support Programme / Respiratory Clinic
Asthma affects 600,000 Kiwis, according to the NZ Asthma Foundation, and symptoms are often triggered by allergens, some medicines, physical activity, smoke, flu and weather changes. Asthma disproportionately affects Maori and Pacific people, and lower socio-economic groups. Studies have linked socio-economic disadvantage with difficulty in accessing primary health care needs, leading to hospital admission for asthma.

Diabetes Management

The Fono offers support to patients who have ongoing long-term chronic diabetes, including support and follow up when you leave the clinic. The Fono team can contact you with reminders when you are due to renew or start new medications and work with you to identify any barriers to health, explain any terminology, condition or treatment that you do not understand.

Heart Disease / Stroke
Heart disease covers all the diseases that affect the heart, including diseases of your blood vessels, such as coronary artery disease; heart rhythm problems (arrhythmias); heart infections; and heart defects you're born with (congenital heart defects).

High Blood Pressure Management
Anyone can develop high blood pressure (also known as hypertension). High blood pressure can increase your risk of heart attack or stroke. High blood pressure usually has no warning signs. Therefore, it is important to have regular blood pressure checks. The Fono offers support and advice to help you manage your high blood pressure and take control of your health. For patients who have ongoing long-term high blood pressure, your doctor is likely to prescribe medication, which can help to control your blood pressure and protect your heart.

The term lung disease refers to many disorders affecting the lungs, such as asthma, COPD, infections like influenza, pneumonia and tuberculosis, lung cancer, and many other breathing problems. Some lung diseases can lead to respiratory failure. The Fono offers support to patients who have an ongoing long-term chronic lung disease condition, including support and follow up when you leave the clinic.
