Family Support - Waitemata DHB

The Family Support programme aims to provide and facilitate additional care and support services to infants, children, young people and their families to decrease re-admission rates to hospital.

The programme focuses on a shared responsibility amongst government and community agencies to facilitate on-going services to infants, children and young people and their families.

The service includes:

  • Providing access to health services, education and resources so that service users have opportunities to make healthier choices
  • Appoints a trained community health worker who completes an assessment to develop a relevant care plan with the family
  • Helps to access a wide range of services such as relationship services, counselling services and parenting services
  • Assists to support immediate needs such as WINZ benefits, housing, employment and budgeting

Eligibility
Patients who are enrolled with The Fono and have a child under 15 years. NOTE: Families are not eligible if enrolled with Waitakere Family Start.
