About the Role

As an Outreach Administrator, you will be an important part of our multidisciplinary outreach team, working alongside healthcare professionals and community teams to improve patient engagement and access to services.

  • Maintaining accurate records, databases and reporting information.
  • Supporting patients who may face barriers to accessing healthcare.
  • Working alongside nurses, social workers, health coaches, kaiāwhina and community partners to provide coordinated support.

Your work will help ensure patients do not miss important opportunities for care and are connected with the right support at the right time.

  • Confidence using systems, databases and Microsoft Office applications.
  • The ability to manage multiple priorities and follow through on commitments.
  • A commitment to improving health equity and supporting better outcomes for underserved communities.
  • Experience with patient management systems such as Medtech, primary healthcare environments, outreach services or community programmes would be an advantage.
  • Pacific language skills and an understanding of Pacific models of care are highly valued.
