Team Leader - Outreach Team

Lead a dedicated outreach team delivering culturally responsive services that improve access to healthcare and health outcomes for our communities.

About the Role

Reporting to the Outreach Manager, you will provide day-to-day leadership, professional support and operational oversight to a multidisciplinary team including Registered Nurses, Health Coaches, Kaiāwhina and Outreach Administrators.

You will play a key role in ensuring our outreach services are culturally responsive, community-focused, and achieve meaningful outcomes for the people we serve.

Based at one of our Auckland sites (Henderson, Blockhouse Bay or Manurewa), this role involves working closely with staff, patients, whānau, community partners and healthcare providers to ensure services are accessible, effective and aligned with The Fono's commitment to equity.

Leading and supporting a high-performing outreach team.
Coaching and developing staff to achieve their full potential.
Supporting service delivery and ensuring contractual targets and KPIs are achieved.
Strengthening systems, reporting processes and continuous improvement initiatives.
Promoting culturally safe, patient-centred and whānau-centred practice.
Building strong relationships with community organisations, healthcare providers and stakeholders.
Supporting innovative approaches that improve engagement and health outcomes for Pacific and Māori communities.

About You

You are a confident people leader who is passionate about community wellbeing and making a positive impact.

You will bring:

A relevant qualification in health, social services, community development or a related field.
Experience leading multidisciplinary teams.
Strong understanding of primary healthcare, outreach services and community-based support.
Knowledge of Pacific and Māori health equity issues and the factors influencing access to care.
Proven ability to support staff performance, wellbeing and professional development.
Excellent organisational, communication and relationship management skills.
A commitment to culturally responsive and equity-focused service delivery.
A current full New Zealand Driver Licence.
